OSHA requires employers to keep records of injuries and illnesses that take place on the job or in the workplace. While certain employers are exempt from such OSHA recordkeeping practices, (those with fewer than 10 employees) most must use what is know as the OSHA 300 Log to keep their records.

The OSHA 300 Log is the record of all workplace injuries and illnesses, and there are specific OSHA guidelines as to which types of injuries and illnesses must be reported. Generally the guidelines specify that all incidents that are work-related or affect work being done by or the health of an employee be reported. Employees are also responsible for reporting accidents to their employer and being sure a record is kept.
